Hunterdon Healthcare Foundation Receives $15K Major Grant from The Provident Bank Foundation

Hunterdon Healthcare Foundation today announced it has been awarded a Major Grant in the amount of $15,000 from The Provident Bank Foundation to support their Transportation Access initiative.

Hunterdon Healthcare Foundation was selected to receive the Major Grant based on their project’s ability to address immediate needs, create meaningful impact, and lead to sustainable community enhancement. The Transportation Access initiative provides free transportation for patients in need so they are able to get to and from Hunterdon Medical Center and outpatient offices.  Care Coordinators arrange transportation for this population so important medical appointments are not missed or patients have safe and appropriate rides home.

“The individuals helped by this program have complex medical conditions and are fragile patients who require extra assistance.  We are grateful to receive this grant that will help us meet critical, high risk patient needs and reduce the likelihood of unnecessary hospitalizations and readmissions,” said Nicole Camporeale, Administrative Director of Clinical Operations & Compliance, Hunterdon Healthcare Partners.
